What health insurances do you accept?
We are currently in-network with the following insurance providers:
Anthem, Medicare, Multiplan, CIGNA, Medicaid, Tricare, Aetna, United Healthcare / Optum Physical Health, Healthkeeper

What do I need to schedule my physical therapy evaluation?
You will most likely need a signed referral from a physician in order to start physical therapy. The referral must be signed within 30 days of your physical therapy evaluation. Please call our office if you do not have a primary care physician. You will also need a copy of your insurance card and form of identification at the time of your initial physical therapy evaluation.
How long will I have to wait to schedule my physical therapy evaluation?
After you call our office to schedule your initial evaluation, we will work with you quickly to give you the next available appointment.
